How to Receive Your Rebate!
- Get an eye exam. Product purchase must be made within 90 days of eye exam.
- Purchase required quantity of FreshLook®, Focus® Softcolors®,
Focus® DAILIES®, NIGHT & DAY®, or O2OPTIX™ contact lenses as indicated
above.
- Complete the mail-in certificate.
- Attach your original dated sales receipt (photocopies are not acceptable),
for the qualifying purchases and a copy of your valid eye exam receipt.
- Mail in the package UPC codes from half of the product purchase as
indicated above, along with your original sales
receipt indicating total purchase quantity (photocopies are not
acceptable), a copy of your eye exam receipt, and this Official

Terms and conditions:
Purchase must be made between January 1, 2008 and July 31, 2008 and received at the above address
by August 31, 2008. Product purchases must be made
within 90 days of exam. Offer is limited to one rebate per person. Offer is not valid in combination
with any other offer or rebate. Photocopy of the rebate certificate is not valid for redemption.
Allow 6 - 8 weeks for redemption. No P.O. Boxes, only street and rural addresses are acceptable.
