How to Receive Your Rebate!

1. Get a contact lens fitting and/or eye exam.
2. Purchase required boxes as indicated above.
3. Complete the mail-in certificate.
4. Attach your original dated sales receipt (photocopies are not acceptable), for the qualifying purchases and a copy of your valid eye exam and/or contact lens fitting receipt.
5. Mail in the originial package UPC codes as indicated above next to the product purchased, your original sales receipt indicating total purchase quantity, a copy of your original eye exam or contact lens fitting receipt and this Official Mail-In-Certificate to:

6. Keep a copy of your paperwork for your records.
