How to Receive Your Rebate!

1. Get a contact lens fitting and/or eye exam.
2. Purchase required boxes as indicated above.
3. Save your boxes and all receipt(s) for rebate submission.
4. Go to lensrebates.alcon.com and enter your Indiviual Rebate Code.
5. Download and complete the Alcon Offer Rebate Form.

Valid on purchases July 1, 2012 through January 31, 2013. Rebate submission must be postmarked by February 28, 2013. Only available while supplies of rebate forms last.
