How to Receive Your Rebate!
- Get a contact lens fitting and/or eye exam.
- Purchase required boxes as indicated above.
- Save your boxes and all receipt(s) for rebate submission.
- Go to lensrebates.alcon.com and enter your Indiviual Rebate Code.
- Download and complete the Alcon Offer Rebate Form.
Valid on purchases December 15, 2012 through July 31, 2013.
Rebate submission must be postmarked by August 31, 2013. Only available while supplies of rebate forms last.
Terms and conditions: Purchases must be made between December 15, 2012 and July 31, 2013 and received at the above address by August 31, 2013. Product purchases must be made within 90 days of exam.
