1. Get an eye exam and/or contact lens fitting.
2. Purchase the required boxes of contact lens products as indicated between May 1, 2018 and August 31, 2018.
3. Go to www.rebate.alconchoice.com and select your contact lens brand to receive your Rebate Code.
4. Complete rebate submission process on www.rebate.alconchoice.com and electronically upload or mail the following required documents (you will receive the mailing address when you begin the rebate submission process on myalconchoice.com):
   1. Original sales receipt with DAILIES TOTAL1 or DAILIES AquaComfort Plus products circled
   2. One [1] original UPC code for each brand of lens purchased
   3. Copy of eye exam receipt and/or contact lens fitting receipt
